Tank Size: 5 gallons

Filtration (include media types i.e. activated carbon, floss, etc): filter and foam pad by Fluval.

How long has your tank been set up: 1 year

Using a liquid reagent test kit, please provide the following:

Ammonia: 0.25 ppm
Nitrites: 5 ppm
Nitrates: 2 ppm
PH: 8.0

Temperature: 73 C

Substrate: gravel

Décor/Live or fake plants: couple fake ones (has been used in tanks for a while) and a couple different live ones (not sure the species)

Maintenance (water changes-how much, how often, vacuum, etc): water changes approximately 10% monthly, vacuum

Dechlorinator: yes

Who lives in your tank? Include a  listing of inhabitants, feeding schedule, and how long they have had the inhabitants.

6 Neon tetras, 5 months
1 Amano shrimp, 3 months
A bunch of snails
Fed once daily, pinch of fish flake food

The cats drink out of it, because they like flavored water apparently.

What symptoms are being exhibited and when did they start: I noticed a couple of days ago that one of the neon tetras in the tank appeared to be covered in mucus bubbles. I think another fish has some smaller bubbles on him as well. The fish's behavior seems surprisingly normal, but it's definitely got something going on.

Picture: I would have linked/ added the picture I took, but got the error "Sorry, you need ten post to be able to add links or photos."
